Proof of Residency (Voter's ID/Stub/Certification of applicant/Voter's Id of Immediate Family Members/Voter's ID of Lessor or Photocopy of Real Property Tax)

Diploma/Transcript of Records/ALS Certificate/Form 137/Form 138
