DepEd: End of Course Evaluation | Survey Form for Learning Delivery Modalities Courses in preparation for the School Year 2021-2022

DepEd has released the End of Course Survey or Evaluation forms which are used to capture  the experiences of Teacher, School LAC Leader, School Head, and Supervisor in implementing their chosen LDMs.

We are celebrating with you in successfully hurdling a very challenging year! We honor your hard work in educating our students despite the pandemic by maximizing the various learning delivery modalities. Since we will still be employing distance learning this coming school year, we would like to partner with you in developing efforts to facilitate learning in the new normal.

This End-of-Course Evaluation captures your experiences as a Teacher, School LAC Leader, School Head, and Supervisor in implementing your chosen LDMs. It will inform us on how we can enhance the LDM Courses so they can prepare you in your teaching during the pandemic.
