Download DepEd Modified Basic Education Enrollment Form for School Year 2021-2022

The Department of Education, Early Registration started on March 26 until April 30, 2021.

Incoming Kindergarten, Grades 1, 7, and 11 learners can now register for SY 2021-2022 via remote or in-person registration.

Here are the following methods on how to get a copy of the Modified Basic Education Enrollment Form and how to submit.

1.Parents can can email, chat, text, or call the school to provide the requested information.

2. School Forms can be submitted to Early Registration Desks in schools. Only parents and guardians are allowed to do this.

3. Parents and guardians can get and submit forms to dropboxes in barangay halls.
